  It can be relied on that any date in this format from a US institution is MM/DD/YYYY. Would be nice if the ISO standard YYYY/MM/DD was used.

  I'm not so sure; I've seen lots of dates in 28-June-2015 form (although admittedly most with alpha form of month rather than numeric).

  Further thought: in most cases of ambiguous date, I bet an experienced observer can tell which date was meant based on the specimen
