How do you list these on the form provided? Could someone explain the difference between the three?

The scientific name is Genus species, which (given a little help from http://www.theplantlist.org/) is Scutellaria elliptica. I'm using the later "determination" from the pencil-written note above the main label. Labiatae is the mint family, but we're not supposed to record that, so ignore it. What looks like "Mx" after the species on the main label is an abbreviation of the "author"; again, ignore that. That just leaves Locality. The rest of the information we want isn't given, so there's nothing to do but click on Skip for those fields.
