I couldn't get the #collector 's name: CHK n--dy ?
I can't read the #year. 1940?
Record 19-14 says "Passer Montanus Hepatious" but after Googling I think the last word should be "Hepaticus". I left it as written anyway.

The 2nd last entry (617) has a bird called "zonpeginthus guttatus" I Googled it and didn't come up with anything. Any idea what it is?
