No, rest is just a description of the sample. I'd enter USA - Colorado, skip the rest of the fields (put "not shown" for the date).

Dolly Sods and Tucker County are in West Virginia, not Virginia as the label says!
Here is another example with multiple species: Should they all be entered into the same field? And how about multiple collectors?
