How should quotation marks indicating to repeat what's above (ditto marks) be transcribed? Do I just put " " " or should I enter what the original writer intended to copy?

I think each of these lines will become a separate record in the database, so you should enter the original name rather than just quotation marks. Records will probably sometimes be viewed in isolation from the others on the same page, and quotation marks will be meaningless in that case.
