Newer date than form-field allows

  The plant is dated 1998, but your form field only allows dates up to 1995. Field was left blank as a result.

  I mentioned this in a similar post earlier today. But I have found that I COULD enter a year well past the '1995' in the drop down list (2001 ! ). As a matter of fact, I could enter the entire Month/Day/Year manually (one at a time, of course). One must STILL click the "OK" button, to complete the date entry, however.
